Amedisys in Florida is seeking a Registered Nurse Case Manager to coordinate patient care at home, conduct assessments, and develop plans of care with the care team. You will supervise LPNs and HHAs, perform on-call duties, and ensure thorough, timely documentation to support patient recovery at home.

Responsibilities include referrals, patient education, and ongoing clinical development, with an emphasis on promoting independence and ensuring high-quality outcomes.

Qualifikationen

• 1+ year of clinical RN experience; may require VP approval if <1 year.
• Current RN license for the assigned state(s).
• Current CPR certification and valid driver's license.

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs and documents rationale.
• Promotes patient health and independence through teaching and rehabilitative measures; supervises LPNs and HHAs.
• Completes timely and accurate documentation and communicates progress to the clinical manager.
• Plans staff education and participates in on-call responsibilities.
