Job summary

Help At Home, LLC is seeking a Registered Nurse (RN) Case Manager to support patients living at home. The role focuses on care coordination, documentation, and regulatory compliance to ensure safe and quality home health services.

The position includes education for patients and families, collaboration with field staff, and ongoing process improvements. A GA RN license and CPR/First Aid certification are required, along with a minimum of one year in clinical nursing.

Qualifications

• Active Georgia RN license required.
• CPR/First Aid certification required.
• One year of clinical nursing experience preferred.

Responsibilities

• Monitor patient care plans and ensure compliance with home health standards.
• Coordinate admissions, discharges and transfers with complete medical records.
• Assign cases and manage schedules with field staff to ensure timely patient visits.
• Educate patients and families on home health services, treatment plans and medications.
