The ideal candidate holds an Associate or Bachelor’s degree in nursing, a Texas RN license, and CPR certification, with ability to travel up to 50% and work a flexible schedule in home health settings.

Formación

• Current, unencumbered Texas RN license.
• CPR certification is required.
• Experience in home health care is preferred.

Responsabilidades

• Perform initial and ongoing comprehensive nursing assessments of patient needs at home.
• Implement the plan of care and revise as necessary.
• Educate patient and family about disease, self-care, nutrition, end-of-life needs.
• Coordinate care with the interdisciplinary team and referring agencies.
• Document progress notes and coordinate services based on acuity and patient needs.
• Communicate changes in patient status to physicians and team members.
