Paycom - ATS is seeking a skilled Registered Nurse to deliver in-home care across Leander, TX. You will assess patients, coordinate with the IDT, and implement a tailored Plan of Care to meet evolving needs in a home setting.

You will educate families, perform nursing duties, and document progress while adapting care to patients of all ages and conditions. A valid TX RN license and CPR are required.

Qualifications

  • Associate or Baccalaureate degree in nursing.
  • Current, unencumbered Texas RN license.
  • Current CPR certification.
  • Reliable transportation.
  • Ability to work in patients' homes in various conditions; travel locally.

Responsibilities

  • Provide nursing care for clients in their homes and coordinate care with the interdisciplinary team.
  • Assess the impact of terminal disease on physical, functional, psychosocial, and environmental needs.
  • Implement and revise the individualized Plan of Care (POC).
  • Educate patient and family on disease process, self-care, end-of-life care, nutrition and dietary needs.
  • Initiate preventive and rehabilitative nursing procedures.
  • Prepare clinical and progress notes documenting progress toward goals.
  • Coordinate services with patient, family, and IDT; communicate changes to physician.
  • Determine scope and frequency of services based on acuity and needs.
  • Assess caregiver ability to meet needs and arrange support as needed.

Job Details: Job Location: 202 - JOL Home Health Leander - Leander, TX, Salary Range: $90000.00Salary/year, Primary functions are to provide nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family, and referring agency; and assume the responsibility for coordination of care.

Essential Functions:

  • Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient's physical, functional, psychosocial, and environmental needs and ADLs, i.e., the risk for grief, cultural and spiritual, verbal and non-verbal.
  • Implement the individualized POC and recommend revisions to the plan as necessary.
  • Consulting with and educating the patient and family regarding the disease process, self-care techniques, end-of-life care, nutrition, and dietary needs. Providing training to other staff as needed.
  • Initiating appropriate preventive and rehabilitative nursing procedures.
  • Preparing clinical and progress notes that demonstrate progress toward established goals.
  • Coordinating all patient and family services and prioritizing needs with the members of the IDT.
  • Informing physician and other personnel of changes in the patient's needs and outcomes of the intervention, while evaluating patient/family response to care.
  • Determining scope and frequency of services needed based on acuity and patient/family needs.
  • Assessing the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care.
  • Evaluating own needs for support and using identified systems to meet the need

Qualifications: Job Expectations:

Ability to work in patients' homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases. Must be able to work on a flexible schedule and have the means to travel locally. Some exposure to unpleasant weather.

Qualifications

  • Associate, or Baccalaureate degree in nursing
  • Current, unencumbered license to practice Registered Nursing in Texas
