Job summary
Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will interact with other medical disciplines, perform admission visits, assess eligibility, and develop the plan of care to be followed.

Qualifications
- Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
- Current state license as a Registered Nurse.
- Current state Driver’s License and reliable transportation.
- One year’s experience as a Registered Nurse (two preferred).
- CPR and Hepatitis consent/declination on file.
- Must pass State regulated Criminal History Background check.
Responsibilities
- Coordinate all aspects of the patient’s home care.
- Interact with other medical disciplines as needed.
- Perform admission visit to the patient in the home and determine eligibility for home care services.
- Develop the plan of care to be followed.
