Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager to oversee patient care at home. You will perform admission visits, assess eligibility, and develop a comprehensive plan of care in collaboration with the patient and family.

The role requires a nursing degree (Diploma/Associate/Bachelor), an active RN license, a valid driver's license, CPR, Hepatitis consent, and reliable transportation. We offer competitive pay, benefits, and opportunities for growth.

Qualifications

Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
Current state license as a Registered Nurse.
Current state Driver’s License.
One year’s experience as a Registered Nurse, two preferred.
Proof of CPR and Hepatitis consent/declination.
Reliable transportation with valid and current auto liability insurance.

Responsibilities

Coordinate all aspects of the patient’s home care.
Interact with other medical disciplines as needed and provide professional care to the patient.
Perform admission visit to the patient in the home, determine eligibility for home care services, and develop the plan of care.
