Job summary

Interim HealthCare Inc. is seeking a Home Health RN Case Manager in Montana. You will design and oversee patient care plans, coordinate with physicians and a team of LPNs, CNAs and aides, and promote high-quality home health outcomes. Schedule flexibility and independent work style are emphasized.

The role includes travel reimbursement, after-hours/weekend pay differential, uniform allowance, and 1:1 nurse-to-patient ratios with a focus on work-life balance.

Qualifications

• Active Montana RN license.
• Graduate of an accredited practical nursing program.
• 3–5 years RN experience, preferably in home health.
• CPR certification and knowledge of home health regulations.
• Excellent communication and compassionate patient care skills.

Responsibilities

• Provide home-based care to patients after hospital discharge or during care at home.
• Oversee LPNs, CNAs, and aides delivering patient care.
• Conduct assessments and admissions; develop care plans.
• Oversee clinical staff in the Butte market and surrounding area.
• Educate patients on care plans, medications, therapy, diet and exercise.
• Coach family members on care plans and home safety.
