Homewatch Caregivers of St Cloud Minneapolis St Paul is seeking a dedicated RN Case Manager to coordinate and deliver in-home nursing care. The role includes assessing clients, planning care, and acting as liaison among care teams in client homes and local offices, with occasional remote work as needed.

Qualifications

• Valid and unrestricted RN license in Minnesota.
• BLS certification & CPR required.
• Oasis experience not mandatory but preferred.
• Strong clinical skills and ability to conduct comprehensive assessments.
• Minimum one year of nursing experience (home health or hospice preferred).

Responsibilities

• Perform comprehensive assessments and maintain individualized care plans.
• Coordinate with providers and supervise unlicensed staff.
• Complete reauthorizations and maintain accurate documentation.
• Provide health education and support to clients and families.
• Deliver hands-on nursing care (vital signs, MARS/TARS, discharge planning, etc.).
