PASCO/SW Home Health is seeking a dedicated Home Health RN to deliver clinical expertise and compassionate care directly into patients’ homes. You’ll build strong relationships, assess needs, and coordinate comprehensive care plans with an interdisciplinary team.

Qualifications

• Currently licensed as a Registered Nurse (RN) in Colorado.
• 3 years nursing experience preferred (Home Health or Med-Surg a plus).
• Strong clinical and patient-centered care skills.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.
