Qualifications

• Minimum of 1 year of experience as a Registered Nurse.
• Current CPR/BLS Certification is required.
• Experience with Palliative Care preferred.

Responsibilities

• Initiate, develop, and implement the patient Plan of Care (POC).
• Communicate necessary revisions to patient status with the care team.
• Provide complete and timely documentation consistent with the plan of care.
