Job summary

University of Michigan Health-Sparrow seeks a Home Care Nurse to provide direct nursing care to patients in their homes. The role includes planning and implementing patient care with physicians, and supervising home health staff to ensure safe, effective treatment.

The ideal candidate will have an active RN license in Michigan, a valid driver's license, auto insurance, and BLS certification. Day shift with flexible scheduling is available; weekend rotation may apply.

Qualifications

• Current RN license in Michigan
• Valid Driver's License
• Proof of Auto Insurance
• Active American Heart Association BLS Certification upon hire

Responsibilities

• Assess patient’s condition and initiates appropriate nursing and/or medical care.
• Develops and implements a plan of care with patient and physician.
• Observe, assess and report signs/symptoms to physician and team.
• Demonstrates, teaches, and supervises patient, family, and caregivers.
• Evaluates patient response and reassesses needs regularly.
• Completes required documentation per agency policy.
• Supervises Home Health Aide and Nursing Care Plan implementation.
• Maintains equipment, supplies, and safe environment.
• Keeps knowledge current with nursing home care trends.
• As RN Case Manager, ensures plan of care communication and visit planning.
