University of Michigan Health-Sparrow is seeking a Registered Nurse to provide direct nursing care, treatment, and health instructions to patients in their homes in Lansing, MI. The RN will collaborate with physicians and families to implement and monitor a patient-centered plan of care.

The role requires Michigan RN licensure, a valid driver's license, AHA BLS certification, and the ability to work independently in home settings.

Qualifications

• Active RN license in Michigan.
• BLS certification upon hire.
• Valid driver's license.
• Proof of auto insurance may be required.
• No experience required for per diem; RN in acute/long-term/home care if applying per diem.

Responsabilités

• Assess patient condition and initiate appropriate nursing care.
• Collaborate with physician and patient to plan care.
• Monitor patient’s status and report changes to the team.
• Demonstrate and supervise care procedures with patient and family.
• Update care plans and reassess needs regularly.
• Complete required documentation per agency policy.
• Manage Home Health Aide and Nursing Care Plans.
• Maintain equipment, supplies, and a safe home environment.
• Ensure compliance with state, CMS, and Joint Commission standards.
