Job summary

Freedom at Home is seeking compassionate Registered Nurses (RNs) to join our growing home health team in Michigan. We offer Full-Time, Part-Time, and PRN opportunities with excellent pay, paid mileage, and flexible scheduling to support work-life balance.

As an RN, you will perform skilled nursing visits, develop plans of care, complete OASIS and clinical documentation, and collaborate with physicians and the interdisciplinary team to ensure high-quality patient care at home.

Qualifications

• Graduate of an accredited Registered Nursing (RN) program.
• Current, unrestricted Ohio Registered Nurse license.
• Current CPR certification.
• Minimum of one year of nursing experience.
• Home Health experience is preferred but not required.
• OASIS experience is a plus.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Perform skilled nursing visits, assessments, admissions, recertifications, resumptions of care, and discharges in patients' homes.
• Develop, implement, and update individualized Plans of Care.
• Complete OASIS assessments and clinical documentation accurately and on time.
• Monitor patient progress and communicate changes with physicians and the interdisciplinary team.
• Educate patients and caregivers on disease management, medications, treatments, and safety.
• Coordinate care with physicians, therapists, home health aides, and other healthcare professionals.
• Supervise Home Health Aides in accordance with state and federal regulations.
• Promote safe, compassionate, high-quality patient care while helping patients remain safely at home.
