Job summary

Phoenix Home Care & Hospice is seeking a Home Health Case Manager to coordinate in-home patient care, supervise LPNs, and collaborate with a multidisciplinary therapy team in the Lake Ozark area. You will develop personalized care plans aligned with physician orders and perform thorough initial assessments.

Requirements include MO or Compact RN license and a valid driver’s license; Home Health or Critical Care experience is preferred.

Qualifications

• Must hold Missouri or Compact RN license and valid driver's license.
• Home Health or Critical Care experience preferred.
• Driver's license and vehicle with current auto insurance.

Responsibilities

• Manage caseload including supervision of LPN visits.
• Coordinate visits, evaluations, and starts of care with patient interaction.
• Educate patients on health conditions such as CHF, COPD, diabetes, wound care, and medications.
• Communicate with physicians to obtain orders and write care plans.
• Collaborate with PT/OT/Speech therapy teams.
