Masonicare Home Health & Hospice is seeking a dedicated Registered Nurse Case Manager based in Wallingford, CT. This full-time visiting nurse role focuses on delivering high-quality nursing care in the home setting, coordinating care with an interdisciplinary team, and supporting clients and families.

Qualifications

• Minimum 1 year clinical experience is required.
• New grads will not be considered.

Responsibilities

• Provide quality and safe delivery of nursing services to patients in the home setting.
• Develop, implement and evaluate the plan of care and revise as needed.
• Coordinate the plan of care and maintain continuity of client care with other health professionals.
• Supervise LPNs and Home Health Aides in provision of delegated duties and conduct supervisory visits.
