Job summary

Dartmouth Health Home Care is hiring an RN Case Manager to deliver skilled nursing services in patients' homes. As part of an interdisciplinary team, you will assess needs, plan care, and coordinate services for patients and families in the Upper Valley region.

This part-time role (20 hours per week) offers meaningful work, flexible scheduling, and opportunities to support patients across multiple towns. A Vermont or NH RN license (or compact) and home health experience are preferred.

Qualifications

• Must be a licensed RN in Vermont and New Hampshire or hold a compact license.
• Graduate of an accredited school of nursing
• One year of experience as an RN in a home health care setting is preferred

Responsibilities

• Screen, evaluate, re-evaluate, and treat patients referred by a licensed provider.
• Coordinate treatment activities with health care team and patient.
• Develop and review care plans with physicians, patients, and families.
• Provide ongoing assessment of clinical, functional, and emotional status.
