Job summary

Dartmouth Health Home Care is seeking an RN Case Manager to join our team in the Upper Valley region. This part-time role (20 hours per week) focuses on coordinating patient care, developing care plans, and working within an interdisciplinary team to deliver high-quality home health services.

The successful candidate will be a licensed RN in VT or NH (or a compact license) with prior home health experience preferred. We offer competitive benefits and a supportive work environment.

Qualifications

• Must be a licensed RN in Vermont or New Hampshire or hold a compact license.
• Graduate of an accredited school of nursing
• One year of experience as an RN in a home health care setting is preferred

Responsibilities

• Develops a care plan in collaboration with the patient’s physician, the patient, and their family or primary caregiver upon admission.
• Periodically reviews the patient care plan and recertifies the plan as appropriate.
• Coordinate the patient’s care plan through interdisciplinary communication and case conferences, as appropriate.
• Provide ongoing assessment of the patient’s clinical, functional, and emotional status and communicate changes to the physician and colleagues.
• Document evaluations, treatment goals, and objectives.
• Teach the patient and caregiver self-care techniques and provide instructions as ordered by the provider.
• Provides clinical oversight of LPNs and LNAs in the home.
