Sutter Health’s SCAH-Sutter Care at Home - Bay seeks a Primary Care Clinician to lead an interdisciplinary team in delivering high-quality home health care. The role emphasizes guidance to RNs, LVNs, PTAs, OTs, COTAs, SLPs, MSWs and HHAs, with focus on patient-centered planning and care management.

Applicants must be a California-licensed RN with current BLS, a valid driver’s license, and access to a vehicle.

Qualifications

• RN license and California registration required.
• DL—Valid Driver’s License required.
• BLS—Basic Life Support Healthcare Provider required.
• Access to a reliable automobile.
• Must meet productivity standards as indicated by supervisor.

Responsibilities

• The Primary Care Clinician reports directly to the Clinical Manager.
• May give functional guidance to Team RNs, LVNs, PTAs, OTs, COTAs, SLPs, MSWs and HHAs.
• Demonstrates leadership and negotiates resources for cost-effective patient care.
• Exhibits independent judgment in planning and providing patient care.
