Job summary

Amedisys Home Health is seeking a Registered Nurse for full-time days to support patients at home. You will perform patient assessments, collaborate with the care team to develop and implement a plan of care, promote health and independence, and coordinate referrals as needed to ensure seamless home-based treatment.

Ideal candidates have 1+ year RN clinical experience, an active RN license in the assigned state, CPR certification, a valid driver's license and reliable transportation.

Qualifications

• 1+ year of clinical RN experience.
• Active RN license in assigned state(s).
• CPR certification.

Responsibilities

• Performs patient assessments and collaborates with care team to develop and implement a plan of care.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to clinical manager.
• Plans staff education and participates in on-call responsibilities.
