Job summary

Crossroads Hospice & Palliative Care seeks a Registered Nurse Case Manager to provide direct patient care in homes or care facilities. You will listen to patients' needs and respect their wishes, building trust with families as the primary link within our holistic care team.

The role emphasizes compassionate end-of-life care with a Monday–Friday schedule, no mandatory on-call, and a comprehensive benefits package including health, dental, vision, 401k and PTO.

Qualifications

• Six (6) months minimum prior skilled nursing experience.
• Active, unencumbered state Registered Nurse license.
• Current driver's license and auto liability coverage.
• Interest and Ability in providing patient care in homes and facilities.

Responsibilities

• Provide direct patient care in patients' homes or care facilities.
• Serve as the primary communication link within the interdisciplinary team.
• Support patients and families by listening to needs and respecting their wishes.
