Job summary

Crossroads Hospice is seeking a Registered Nurse Case Manager to provide direct care to patients in their homes or care facilities. You will serve as the primary liaison, ensuring compassionate end-of-life care that respects patient wishes and supports families.

Joining our team means working Monday to Friday, 8:30 AM to 5:00 PM, with no mandatory on-call and a benefits package including health, dental, vision, 401k, and PTO.

Qualifications

• Six (6) months minimum prior skilled nursing experience.
• An active, unencumbered state Registered Nurse license
• Current driver's license and auto liability coverage
• Interest and Ability in providing patient care in homes and facilities

Responsibilities

• Provide direct care to patients in their homes or care facilities.
• Communicate with patients, families, and multidisciplinary team to coordinate care.
• Grow within a team that values listening, being heard, and professional development
