Corewell Health Hospice is seeking a Nurse Case Manager to coordinate compassionate end-of-life care for patients and families. Under Nursing Manager, you will assess needs, create care plans, and collaborate with an interdisciplinary team to ensure comfort and quality care.

You will travel to patients’ homes, nursing facilities, and hospitals, providing education and support, while maintaining compliant documentation and participating in ongoing professional development.

Qualifications

• Must hold an RN license in Michigan and a valid driver's license.
• Minimum of 2 years of relevant nursing experience.
• Associate's degree in nursing required; bachelor's degree preferred.

Responsabilités

• Assess patients and develop individualized care plans.
• Coordinate end-of-life care and symptom management.
• Collaborate with physicians, social workers, chaplains and team.
• Educate patients and families on disease progression and options.
• Provide emotional and psychosocial support.
• Maintain accurate, compliant patient care documentation.
• Visit patients at home, nursing facilities, and hospitals.
• Seek ongoing professional development in hospice care.
• Participate in safety initiatives and risk management.
• Perform other duties as assigned.
