Amedisys Home Health in Georgia is seeking a registered nurse case manager for full-time days. You will assess patients, develop and implement a plan of care, and coordinate with the care team to promote healing at home.

Responsibilities include referrals, patient education, supervising LPNs and HHAs, timely documentation, and on-call coverage. RN licensure in the state, CPR, and a valid driver's license required. Mileage reimbursement provided.

Qualifications

• 1+ year of clinical RN experience.
• Current RN license for the assigned state.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines as indicated by patient needs.
• Promote health and independence through teaching and rehabilitative measures; teach self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
