Amedisys, Inc. is seeking a Registered Nurse Case Manager for the Suffolk Territory.

This full-time role includes weekend call rotation and a compensation range of $82,000 to $88,000 annually, with a $7,500 bonus noted and eligibility for additional benefits. The position supports home health and hospice services, with orientation followed by a competitive benefits package including medical plans (PPO or HSA) with telehealth, PTO, wellness rewards, mental health resources, and retirement

Qualifications

• 1+ year of clinical RN experience.
• Current RN license for the assigned state(s).
• CPR certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to plan and implement care.
• Makes referrals as needed based on patient needs.
• Promotes patient health through education and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes timely and accurate documentation.
• Communicates patient progress to the clinical manager and team.
• Participates in staff education and on-call duties.
