Job summary

Amedisys, part of the Optum family of businesses, is recruiting a Nurse Case Manager in North Carolina to guide patients and families through the care continuum. You will assess needs, coordinate care, support discharge and transition planning, and collaborate with a Care Advocate and Social Worker to ensure appropriate care levels.

Under supervision of an RN or MD, you will educate, coach, and support treatment decisions while supervising LPNs and HHAs.

Qualifications

• Current RN license valid in the state(s) you are assigned to work.
• Current CPR certification.
• 1+ years of clinical RN experience.
• Valid driver’s license and liability insurance.

Responsibilities

• Assess patient needs and coordinate care along the continuum.
• Collaborate with the care team and supervise LPNs and HHAs.
• Complete timely and accurate documentation.
• Participate in on-call responsibilities and education programs.
• Discharge planning and transition support.
