Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse in Lincoln, NE to provide one-on-one, in-home care under the Clinical Manager. You will work with physicians and clinicians to manage care plans that support recovery and help patients regain independence.

Qualifications

• Med-surg, ICU, ER, or acute experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes, including medications, wound care and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management and treatments.
• Maintain accurate care records and coordinate with the care team.
• Communicate patient progress to physicians and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
