Job summary

CenterWell Home Health in North Carolina seeks a dedicated Home Health Registered Nurse to provide one-on-one nursing care at patients’ homes, under the supervision of the Clinical Manager. You will develop individualized care plans, monitor patient progress, and educate families on disease management and treatments.

The role offers a $10,000 sign-on bonus, 40 hours per week, and a competitive pay range ($49.00–$69.00 per visit) with annual pay range estimated from $77,200 to $106,200 depending

Qualifications

• Current and unrestricted RN licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver’s license and reliable transportation are required.

Responsibilities

• Provide skilled nursing care to patients in their homes, including medications administration, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management and treatment options.
• Maintain patient care records and coordinate with healthcare professionals.
• Report progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
