Job summary

Humana / CenterWell Home Health is seeking a Home Health Registered Nurse to deliver one-on-one skilled nursing care in patients' homes, administer medications, wound care, and comprehensive assessments while coordinating with a multidisciplinary team.

You will develop individualized care plans with physicians, educate families, monitor progress, and oversee Home Health Aides. A valid RN license, CPR, and driving eligibility are required; a sign-on bonus is offered.

Qualifications

• Current RN licensure in the state.
• CPR certification required.
• Experience in Med-surg, ICU or ER settings.
• Ability to collaborate with a healthcare team.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide skilled nursing care at patients' homes, including meds administration and wound care.
• Develop and implement individualized care plans with physicians.
• Monitor patient conditions and report changes promptly.
• Educate patients and families on disease management and treatments.
• Maintain accurate care records and coordinate with clinicians.
• Oversee Home Health Aides and ensure proper documentation.
