Job summary

Amedisys Home Health in Sevierville, TN is seeking a Registered Nurse Case Manager. You will perform patient assessments, develop and implement plans of care with the care team, supervise LPNs and HHAs, and ensure timely documentation and communication with the clinical manager.

Qualifications

• 1+ year of RN clinical experience.
• Active RN license in applicable state.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals as indicated and documents rationale.
• Supervises LPNs and HHAs and ensures timely documentation.
• Communicates patient progress to the clinical manager and care team.
• Participates in staff education and on-call responsibilities.
