Job summary

Amedisys Home Health is seeking an experienced Registered Nurse Case Manager for weekend shifts in Murfreesboro, TN. You will perform patient assessments, collaborate with the care team, and develop care plans to promote patient health at home.

Responsibilities include supervising LPNs and HHAs, timely documentation, and on-call duties. Competitive benefits and growth opportunities are offered with a respected national home health provider.

Qualifications

• 1+ year of clinical RN experience; required unless VP approval.
• Current RN license in applicable state(s).
• CPR certification current and valid.
• Valid driver’s license and reliable transportation.
• If RN experience <6 months, participation in RN Intern program.

Responsibilities

• Perform patient assessments with the care team to develop and implement plans of care.
• Make referrals to other disciplines as indicated and document rationale.
• Promote patient health and independence through teaching and rehabilitation.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Participate in on-call responsibilities and continuing education.
