Job summary

Amedisys Home Health in Tennessee is seeking a registered nurse case manager to join our home-based care team. The role centers on patient assessments, care planning, and coordinating with the care team to ensure high-quality, at-home care.

Qualifications

• At least 1 year RN clinical experience; if less, VP approval required.
• Current RN license for the states you serve.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Note: if <6 months RN experience, participate in RN Intern program.

Responsibilities

• Perform patient assessments and develop plans of care with the team.
• Make referrals to other disciplines as needed.
• Promote health and independence through teaching and rehabilitation.
• Supervise LPNs and HHAs.
• Complete timely and accurate documentation.
• Communicate patient progress to clinical managers.
• Plan staff education and participate in on-call duties.
• Engage in clinical development and continuing education.
• Other duties as assigned.
