Interim HealthCare of Oklahoma City is seeking an experienced Home Health RN Case Manager in Oklahoma City. You will join a full care team and shape the plan of care for your patients, not just complete tasks. The role offers autonomy, meaningful patient relationships, and a voice in care decisions.

Qualifications

• Active RN license in Oklahoma.

• CPR certification required.

Responsibilities

• Develop and adjust the plan of care for patients with the care team (LPNs, therapists, physicians).
• Complete visits, OASIS assessments, and documentation with a patient-centered view.
• Coordinate with the care team to prevent gaps in care.
• Educate patients and families on diagnosis, medications, and home safety.
• Advocate for patients to drive meaningful outcomes.

Schedule
Full time, Monday through Friday, with flexibility to structure your day around your patients.

What You'll Earn
We offer flexible pay structures for this role, either salary or per-visit, whichever fits you best:

• Per-visit option: $40 per visit, plus $100 for every Start of Care, $75 for every Resumption of Care, and $60 for every Recertification
• Salary option available for the right candidate

What We're Looking For

• Active RN license in Oklahoma

• CPR certification
