UT Southwestern Medical Center in Dallas seeks a Field Registered Nurse - Home Health Case Manager to provide comprehensive home health nursing per individualized care plans. You will conduct assessments, coordinate services, and educate patients and families while ensuring high-quality outcomes.

The role requires 2 years RN experience with 1 year adult IV expertise, a Texas RN license (or compact), a valid TX driver's license, and BLS.

Qualifications

• Education: Graduate of an accredited nursing program.
• Experience: 2 years of RN experience with 1 year of adult IV expertise.
• Licenses & Certifications: Texas RN license (or BNE Compact State), valid Texas driver's license, BLS certification.

Responsibilities

• Provide direct nursing care according to the client’s plan of care.
• Perform comprehensive home health assessments including OASIS when appropriate.
• Maintain accurate patient records and med profiles.
• Administer IV fluids, medications, and related therapies as prescribed.
• Perform phlebotomy, specimen collection, and point-of-care testing.
• Ensure high-quality patient care meeting regulatory standards.
• Supervise Home Health Aides and LVNs as required by regulations.
• Participate in on-call rotation and agency CQI activities.

JOB SUMMARY

The Case Manager for Home Health provides nursing care in accordance with the client's plan of care, and includes comprehensive health and psycho-social evaluation, monitoring of client's condition, health promotion and prevention, coordination of services, teaching and training activities and provision of direct nursing care and other duties as deemed necessary.

EXPERIENCE AND EDUCATION

Required

• Education
  Graduate of accredited nursing program.

• Experience
  2 years of RN experience with one (1) year experience with adult IV expertise required.

• Licenses and Certifications
  (RN) REGISTERED NURSE by Board of Nurse Examiners for the State of Texas (or BNE Compact State). and
  (DRIVERS) DRIVERS LICENSE Valid Texas driver's license, proof of driver's insurance. and
  (BLS) BASIC LIFE SUPPORT accredited course by the American Heart Association (AHA) or American Red Cross (ARC)

Preferred

• Education
  Bachelor's Degree - BSN

• Experience
  Other RN experience including high tech and home health expertise preferred.
  Prefer knowledge of InterQual and TMF care criteria.

JOB DUTIES

• Provides direct nursing care, including patient observation and delivery of prescribed treatments. Conducts initial and ongoing comprehensive assessments of home health patient needs, including OASIS assessments at appropriate time points.
• Evaluates outcomes of patient care and identifies discharge planning needs. Consults with other disciplines as required. Adjusts nursing care processes as necessary.
• Maintains patient records and charts to ensure accurate record of treatment, including med profile, care plan, communication notes, etc.
• Provides instructions to patients/families for use of products and services utilizing nursing process to address learning needs.
• Prepares, administers, monitors, and discontinues IV fluids. Accesses IV delivery system (ie. peripheral IV start, implant cath, broviac, etc.).
• Administers medications and treatments as prescribed by the physician.
• Performs phlebotomy, laboratory specimen collection and point of care testing.
• Responsible for overall delivery of quality patient care that meets or exceeds established standards and provides assistance in activities to meet state and/or federal regulatory guidelines.
• Maintains safe, comfortable, and therapeutic environment for patients/families in accordance with established standards.
• Participates in Continuous Quality Improvement (CQI) Program.
• Provides supervisory visits for Home Health Aides and LVNs as required by state and federal regulations.
• Responsible for participating in on-call rotation and emergency call according to agency policy.
• Performs other duties as assigned.
