Job summary

UT Southwestern Medical Center in Dallas, TX is seeking an LVN for home health to provide experienced nurse’s care under physician or nursing leadership. The role supports patient safety, treatment delivery, and documentation within the scope defined by the Texas Board of Nurse Examiners.

The position requires graduation from an approved LVN program, 1–2 years post-high school education, and TX LVN license with BLS. Preference is given to candidates with home health experience.

Qualifications

  • Graduate of an accredited Licensed Vocational Nurse program.
  • 1-2 years of post-high school education.
  • Texas LVN license with BLS certification; driver's license and proof of insurance.

Responsibilities

  • Provides direct nursing care and delivers prescribed treatments.
  • Monitors patient condition and reports changes to the care team.
  • Maintains patient records and charts for accurate treatment history.
  • Educates patients/families on product usage and care through nursing process.
  • Prepares and manages IV fluids and access devices as directed.
  • Contributes to high-quality patient care meeting state/federal standards.
  • Maintains a safe, therapeutic environment and participates in QAPI.
  • Attends in-services and staff meetings; performs other duties as assigned.

JOB SUMMARY

This position works under the general supervision of a physician, advanced practice provider, and/or nursing leadership to provide experienced para-professional nursing skills in treatment, care and safety of patients. Works within their scope of practice as defined by the Board of Nurse Examiners for the State of Texas.

EXPERIENCE AND EDUCATION
Required
  • Education Graduate of an accredited Licensed Vocational Nurse program from approved school of nursing.
  • Experience 1-2 years of post-high school education.
  • Licenses and Certifications (LVN) LICENSED VOCATIONAL NURSE by Board of Nurse Examiners for State of Texas (BLS) BASIC LIFE SUPPORT (DRIVERS) DRIVERS LICENSE TX Driver's License and proof of driver's insurance
Preferred
  • Experience 2-3 years' experience in High Tech and Home care
JOB DUTIES
  • Provides direct nursing care, including patient observation and delivery of prescribed treatments.
  • Monitors medical condition of patients and reports changes to appropriate personnel.
  • Maintains patient records and charts to ensure an accurate record of treatment.
  • Provides instructions to patients/families for use of products and services utilizing nursing process to address learning needs.
  • Prepares, hangs, monitors, and discontinues IV fluids. Accesses IV delivery system; i.e., peripheral IV start, implant cath, vascular access devices, etc.
  • Responsible for overall delivery of quality patient care that meets or exceeds established standards and provides assistance in activities to meet state and/or federal regulatory guidelines.
  • Maintains a safe, comfortable, and therapeutic environment for patients/families in accordance with established standards.
  • Participates in Organizational Quality Improvement Program (QAPI).
  • Participates in in services and staff meetings.
  • Duties performed may include one or more of the following core functions: (a) Directly interacting with or caring for patients; (b) Directly interacting with or caring for human-subjects research participants; (c) Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records); or (d) Regularly maintaining, modifying, releasing or similarly affecting human-subjects research records.
  • Performs other duties as assigned.
