Job summary

BAYADA Home Health Care is seeking a Full Time Admissions Registered Nurse (RN) to join our home health visit team covering Delaware County and areas of West and South Philadelphia. You will work independently within a multidisciplinary team to deliver high-quality skilled nursing and rehabilitative care in patients' homes.

Responsibilities include head-to-toe assessments, wound care, medication reconciliation, and thorough documentation.

Qualifications

• Current nursing license in good standing in Pennsylvania.
• Experience in adult/geriatric care in acute, emergency, home health, hospice, ICU or med-surg environments.

Responsibilities

• Perform comprehensive head-to-toe assessments and identify subtle changes in patient condition.
• Provide skilled nursing and rehabilitative care at patients' homes to support independence.
• Triage and manage urgent clinical situations with real-time decisions.
• Perform medication reconciliation and patient/caregiver education.
• Document wound assessment, care, and outcomes in the patient's chart.
