Qualifications

• 1+ years of Home Health experience preferred.

Responsibilities

• Provide and direct provisions of nursing care to patients in compliance with applicable laws.
• Coordinate total plan of care with health care professionals.
• Ensure high-quality clinical services in accordance with the plan of care.
• Take direction from Clinical Director and Executive Director.
