Optum is seeking a Registered Nurse in Home Health for Waltham, MA. You will provide and direct nursing care to patients in their homes, coordinating with physicians and the care team to ensure seamless, compliant service.

Formación

• Current and unrestricted RN licensure in state of practice.
• Valid driver’s license, vehicle insurance, and access to dependable transportation.
• CPR certification or ability to obtain within 90 days of hire.
• 1+ year Home Health experience preferred.
• Ability to work independently with strong communication and organizational skills.

Responsabilidades

• Initiates and revises the plan of care with physician collaboration.
• Supervises home health aides and licensed personnel as required by state/federal rules.
• Performs supervisory visits as needed and ensures quality of care.
• Completes comprehensive assessments (OASIS) and reconciles medications.
• Documents visits per policy and payer requirements with accurate data entries.
• Communicates with physicians, patients, and families to ensure continuity of care.
