Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license and access to a dependable vehicle or public transport.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Provide and direct nursing care to home patients as prescribed by the physician.
• Coordinate the plan of care with other health professionals to ensure continuity of care.
• Assess patients and determine eligibility and medical necessity for services.
• Develop plans of care with input from physicians and care teams.
