Job summary

UnitedHealth Group is seeking an experienced Registered Nurse to provide and direct in-home nursing care across Connecticut. You will coordinate ca re plans, assess patients, and ensure medical necessity for services while complying with all laws and agency policies.

You will collaborate with physicians and care teams to deliver high-quality care, adapt plans as needed, and support patients and families in managing conditions and goals across the care continuum.

Qualifications

• Current and unrestricted RN licensure in the state of practice (CT).
• CPR certification or ability to obtain within 90 days, preferred.
• Ability to work independently and coordinate care with a care team.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician.
• Coordinate total plan of care with other health care professionals.
• Complete nursing assessments per program requirements and payer needs.
• Ensure eligibility and medical necessity for services defined by payer.
• Develop and revise individual care plans with community providers.
• Monitor plan of care frequency and duration; revise with physician input.
• Maintain clear communication with patient, family, and team.
