• Current and unrestricted RN licensure in state of practice.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and health care team.
• Supervises care provided by home health aides and licensed nurses, provides instruction and assigns tasks per regulations.
• Completes comprehensive assessments (OASIS) including medication reconciliation; documents visits per policy.
• Ensures high-quality clinical services and coordinates care with the healthcare team to meet patient needs.
