Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health to provide and direct nursing care in patients' homes, coordinating the plan of care with physicians and the care team. The role requires strong clinical skills, compliance with laws, and collaboration with families and community resources.

Ideal candidates hold an RN license, CPR certification (or willing to obtain), and have 1+ year of home health experience, plus reliable transportation and excellent communication abilities.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR Certification or ability to complete within 90 days of hire.
• 1+ years of Home Health experience.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician.
• Supervises home health aides and licensed nurses and assigns tasks per regulations.
• Completes comprehensive assessments (OASIS) and medication reconciliation.
