Job summary

UnitedHealth Group is seeking an experienced Registered Nurse for Home Health to provide and coordinate patient care in patients' homes. The RN will work with physicians, the interdisciplinary team, and families to ensure a complete plan of care and continuity of services.

Responsibilities include developing care plans, supervising home health aides, documenting care, and supporting quality improvement while participating in on‑call rotations. A valid RN license and driver's license are required.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Develops and updates patient care plans in collaboration with physicians and the team.
• Provides and coordinates high‑quality clinical home health care and supervises aides.
• Documents care per policy; communicates with physicians, patients, families and staff.
• Participates in orientation and contributes to quality and safety initiatives.
• Adheres to professional standards and participates in on‑call/weekend rotations.
• Function effectively in diverse home settings with respect for all individuals.
