Job summary

Beth Israel Lahey Health is seeking a registered nurse for home health to assess patients, plan care, and supervise aides in patients’ homes. The role emphasizes community health education, HIPAA compliance, and coordination with physicians.

Candidates should hold an active Massachusetts RN license with BLS, have at least 1 year of direct care experience, and preferably a BSN. EPIC and OASIS experience are valued. Salary is competitive with generous benefits and PTO.

Qualifications

• Active Massachusetts RN license.
• American Heart Association BLS certification.
• Associate’s Degree in Nursing or BSN preferred.
• At least 1 year of direct care nursing experience.
• OASIS and EPIC experience preferred.

Responsibilities

• Complete and document an initial patient and family assessment.
• Initiate and revise the plan of care as patient needs change.
• Administer medications and treatments per physician.
• Instruct and supervise home health aide care every two weeks.
• Identify discharge planning needs as part of care planning.
• Maintain HIPAA confidentiality.
• Complete OASIS accurately.
• Document in EPIC per timeliness policies.
