Enhabit Home Health & Hospice in Thornton, CO seeks a skilled nurse leader to coordinate home health services, review start-of-care data, and align patients, physicians, caregivers, and staff for seamless care delivery.

The ideal candidate is a graduate of an approved nursing program, holds a valid clinical license, has 2+ years of nursing experience (including 1 year in home health), CPR, a valid driver’s license, and comfort with Homecare Homebase; bilingual English/Spanish preferred.

Competenze

• Graduate of an approved school of professional nursing.
• Valid clinical license per state requirements.
• Minimum two years of nursing experience within the last 36 months.
• One year of experience in home health.
• CPR certification and valid driver's license.
• Bilingual English/Spanish preferred.
• Office attendance is essential; remote work not allowed.
• Experience with Homecare Homebase (HCHB) preferred.

Mansioni

• Coordinate overall home health services to meet standards and procedures.
• Review and approve patient information during start of care, recertification, or evaluations.
• Facilitate relations between physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and ensure continuous communication for care coordination.
