Job summary

Qualifications

• Registered Nurse with current licensure in MA/NH and an NLN-accredited nursing education.
• BSN degree strongly preferred; related professional certifications are a plus.
• Experience in home health nursing with OASIS knowledge is preferred.

Responsibilities

• Coordinate patient care for a caseload with delegation to LPN staff and monitor patient status.
• Complete initial assessments and develop patient-specific plans of care with family involvement.
• Initiate and revise care plans as patient needs change.
• Administer medications and treatments per physician orders.
• Counsel patients and families; provide health education and discharge planning.
• Precept and supervise new staff; participate in weekend/holiday/on-call rotations.
