Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health in Oak Brook, Illinois, to provide skilled nursing in patients' homes. This staff full-time role includes a 40-hour work week, 8-hour shifts, and travel to patient locations within the assigned territory.

The position offers a total bonus potential of $15,000, including a $10,000 first-year success bonus and a $5,000 sign-on bonus, with eligibility based on performance and status.

Qualifications

• Active RN license in the state you work (Illinois).
• Minimum of 1 year RN experience delivering patient care.
• Valid driver’s license required for travel to patient homes.

Responsibilities

• Deliver skilled, patient-centered home health care
• Perform patient assessments, medication management, and care planning
• Educate patients and families on disease management and treatment plans
• Ensure accurate EMR documentation
• Work independently with support from an interdisciplinary team
• Travel to patient homes within assigned territory
