Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health in Westmont, Illinois, to provide skilled nursing care in patients' homes. The role is full-time (40 hours/week), with an 8-hour shift and ongoing employment.

A $15,000 total bonus potential includes a $10,000 first-year success bonus and a $5,000 sign-on bonus. We offer comprehensive benefits, field support from Day 1, and opportunities for growth within Residential Home Health and Hospice.

Qualifications

• Active RN license in the state you work
• One year RN experience in home health or similar setting
• Valid driver’s license and ability to drive to patient homes (about 20% of work time)

Responsibilities

• Deliver skilled, patient-centered care in patients' homes
• Perform patient assessments, medication management, and care planning
• Educate patients and families on disease management and treatment plans
• Ensure compliant EMR documentation and thought-out care plans
• Work independently with support from an interdisciplinary team
• Travel to patient homes within assigned territory
