The role offers 40 hours per week, 8-hour shifts, with an eligible $10,000 first-year success bonus and opportunities for ongoing career growth within a supportive, field-based team.

Qualifications

• Active RN license in the state you work.
• At least 1 year of RN experience in home health.
• Current driver''s license and ability to travel ~20% of your day.

Responsibilities

• Deliver skilled, patient-centered care in home health setting with interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families on disease management and treatment plans.
• Maintain compliant EMR documentation and think critically.
• Travel to patient homes within assigned territory.
