Nurse On Call, a part of the LHC Group within the Optum family, is seeking a Registered Nurse for home health in Rhode Island. The RN will direct nursing care in patients' homes, coordinate with physicians and care teams, and support care continuity with families and community resources.

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to dependable transportation.
• CPR Certification preferred or able to obtain within 90 days of hire.

Mansioni

• Initiates, develops, implements, and revises the plan of care in collaboration with physicians and health care professionals.
• Supervises home health aides and licensed nurses, assigns tasks per state/federal regulations, and conducts required visits.
• Completes comprehensive assessments (OASIS), medication reconciliation, and accurate documentation per policy.
