Job summary

Nurse On Call in Lincoln, RI is seeking a Registered Nurse (RN) to provide and coordinate home health care for patients in their homes. You will work with physicians, families, and the care team to implement the plan of care, perform assessments, and ensure compliance with regulations.

Qualifications

• RN licensure in the state of practice
• Current driver’s license and access to a dependable vehicle or transportation
• CPR certification preferred or to be obtained within 90 days of hire
• 1+ years of Home Health experience (preferred)
• Ability to work independently and strong communication/organizational skills

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
• Supervises home health aides and licensed nurses, provides instruction, and assigns tasks per state/federal regulations
• Provides supervisory visits and ensures high-quality clinical services and infection control standards
• Completes comprehensive assessments (OASIS) and documents visits per policy and payer requirements
• Communicates with physicians, patients, families, and team to ensure care coordination and positive outcomes
• Participates in on-call and weekend rotation as needed and adheres to utilization management processes
